- Coordinates medical respite (MR) referral/guest process (assessment, program goals, care plan, program guidelines, medication process, intakes, discharges)
- Ensures program policies are adhered
- Maintains up- to-date MR Client Track data for guests
- Maintains up-to-date MR guest files with case notes (paper & electronic)
- Case management lead for MR guests: assess needs/barriers, outline goals and plan for achieving those, interact with support agencies on behalf of clients, provide necessary documents required by outside entities, assist guests w/ job/housing applications, refer to agencies, transport to follow-up appointments, keep meds and med sheet updated and stocked, etc.
- Maintains up-to-date MR inventory record and reporting
- Shift emergency coverage back-up contact
- Oversees and maintains up-to-date MR spreadsheets: Full guest list, shift logs, and service/property suspension list
- Oversees and maintains abandoned guest property according to policy
- Submits and follows up on maintenance requests/needs for MR
- Attends monthly staff meetings/trainings
- Organizes and leads monthly MR task force meetings
- Attends monthly meetings—(Homeless Coalition, 2nd Friday of month)
- Interacts with all levels of staff
- Reports to Director of People and Programs
